We look forward to welcoming you and your family to our practice. At your first visit with us, one of our team members will review your medical and dental histories, insurance information, and complete a comprehensive examination. The doctor will review all treatment options with you, and give you recommendations based on your wants and needs. If time permits, one of our hygienists will be able to clean your teeth that same day. Please fill out the new patient and medical history forms below prior to coming in for your first visit.

We are pleased to be working with dental insurance carriers such as:

We also work with a variety of other insurance carriers on an out-of-network basis, and offer flexible payment options such as Care Credit. Our office accepts all major credit and debits cards including Visa, American Express, Mastercard, and Discover. Please give our office a call to discuss insurance questions and payment options.
